Have You:
- Filled in YOUR mailing address and phone numbers?
- Filled in the name, mailing address, and phone numbers of your SYSTEM?
- Made sure to give your EMPLOYER'S NAME, so we send the results to the right person.
- Checked the box telling us WHERE you want your certification mail sent?
- Marked which classification of exam you want to take? Is it the right classification for your type of system?
- Filled in ALL appropriate experience? If it is not on the application, we cannot count it towards your full certification experience.
- Described the type of system and population you serve?
- Filled in your high school education information? Remember, you MUST have a high school diploma, GED OR request a special exception.
- COMPLETELY filled in your post-secondary education information? Don't forget, it can count up to half of your experience requirement if you have appropriate post-secondary education.
- Marked whether you would like us to notify your employer of your exam results? We will send it automatically if your employer has paid for your fees.
- SIGNED (NOTARIZED) AND DATED YOUR APPLICATION? If you don't sign it, we send it right back!
- Enclosed your application and exam fees? The application is an annual fee and must be paid every year for each application. If you are applying to take both water and wastewater exams, there is an application fee due for each. An examination fee is required for each exam taken.
