SWPPP - Storm Water Pollution Prevention Plan The SWPPP is used to direct and assist permittees in identifying sources of potential pollutants at the construction activity site and Best Management Practices (BMPs ) to be used to help ensure such pollutants do not impact receiving surface waters through storm water runoff. Storm Water Rainfall Erosivity Waiver  This form is to be used for a "storm water discharge associated with construction activity" that results in a construction-related disturbance of less than five acres of total land area, and which results in the construction activity having a Rainfall Erosivity Factor ("R" in the Revised Universal Soil Loss Equation - RULSE) of less than five (5). SW-1 - Storm Water Dischargers This form is used to obtain coverage under a general permit for storm water discharges associated with industrial facilities and mining and oil and gas operations, or that the Department has determined are contributing to a violation of water quality standards or are a significant source of pollutants. Industrial No Exposure Certification Form 

This form is for use by industrial, mining and oil and gas facilities subject to regulation under the Montana Pollutant Discharge Elimination System (MPDES) regulations whose regulated activities and materials are completely sheltered. See ARM 17.30.1116
Industrial Annual Compliance Evaluation Report Form This form is to be completed yearly by all permittees authorized to discharge storm water under the General Permit for Storm Water Discharges Associated with Industrial Activity as required by the permit. Mining and Oil and Gas Annual Compliance Evaluation Report Form and Discharge Monitoring Report This form is divided into three major portions. Permittees may complete and submit the entire form package or only Section One or Section Two along with Section Three. This option depends on the General Permit requirements specific to your facility and/or the time of year. Form 2A

Form 2B Application for permit to discharge wastewater from concentrated animal feeding operations (CAFOs) and aquatic animal production facilities. For CAFOs also see Form NMP. Concentrated Animal Feeding Operation (CAFO)

DMR Example

Discharge monitoring results must be recorded on a DMR form. The permittee shall complete and submit a DMR form for each point source outfall requiring monitoring, including discharges that were not activated. MPDES Permits
