- *2009 Reapplication Form: To be used by existing Small MS4 permittees (authorized under the 2005 General Permit) who will be renewing authorization to discharge under the 2010 General Permit with the exact same permittee and/or co-permittee arrangement as currently exists (no added or removed co-permittees).
- *2009 Application Form: To be used by existing Small MS4 permittees (authorized under the 2005 General Permit) who will be renewing authorization to discharge under the 2010 General Permit with a different permittee and/or co-permittee arrangement as currently exists (co-permittees have been added or removed). This 2009 Application Form is also to be used for new Small MS4 permittees who have not been permitted before.
*Department policy requires original signatures and payment of any required fees before document review can begin.

Note: All maps are based on the US Bureau of the Census 2000 decennial census. For Bozeman, Butte, Helena, and Kalispell, the MS4 regulated area is based on the current city limits, which may be slightly different than these maps indicate.
