OPERATOR CERTIFICATION FORMS

 

  • Drinking Water Application Package - This is a checklist that outlines the requirements to become a certified drinking water operator.

  • Drinking Water Application - Montana application for certification as an operator of a water distribution system or a water treatment system.

  • Wastewater Application Package- This is a checklist that outlines the requirements to become a certified wastewater operator.

  • Wastewater Application - Montana application for certification as an operator of a municipal, industrial, or on-site wastewater treatment system.

  • Temporary Operator Certification Application- Temporary certification may be applied for if the operator in responsible charge of the system is not fully certified to keep the system in compliance. This form must be completed by the system owner.

  • Certification Class Charts - The certification class charts give a brief description of each system classification for drinking water systems and wastewater systems.

  • System/Operator Update Form - This form constitutes a notice of intent from the water or wastewater system listed below to the State of Montana, Department of Environmental Quality (department) for the purpose of providing a means to fulfill the statutory obligation under Section 37-42-302, Montana Codes Annotated (MCA). This MCA requires that all wastewater treatment plants, water treatment plants, or water distribution systems be operated under the supervision of a fully certified operator in responsible charge to ensure the proper management, operation, and maintenance of the system.
