2017 Reapplication Form: To be used by Small MS4 permittees continuing authorizations under the 2015 MS4 General Permit. A renewal application package includes the reapplication form, a storm water management program, and renewal permit fee*.
2017 Application Form: To be used by small MS4 permittees not currently authorized under the 2015 General Permit and seeking authorization under the 2017 General Permit. An application package includes the application form, a storm water management program, and new permit fee*.
Permit Fee Schedule for Applications and Annual Fees
- *Department policy requires original signatures and payment of any required fees before document review can begin.
MS4 Annual Report Form for Calendar Year 2016: To be completed by each permittee or co-permittee authorized to discharge storm water under the MS4 General Permit.
MS4 Annual Report Form for Calendar Years 2017-2021: To be completed by each permittee or co-permittee authorized to discharge storm water under the 2017 MS4 General Permit.
