HELENA -- Silica Mining, Inc, failed to complete reclamation within two years of completion of operations and failed to submit the annual report of activities of Operating Permit #00158 in Beaverhead County, Montana.

The reclamation plan included, but was not limited to, solid waste removal, noxious weed control, demolition of the mill building, removal of mining equipment, and plugging and abandonment of three water wells.
